YOCC Waiver
Pearlside Youth Outreach Community Center Waiver and Release Form
Before participating in any activities at the Pearlside Youth Outreach Community Center (YOCC), a Waiver and Release form must be completed for each individual. A parent or guardian must fill out the form for any minor (17 years or younger). This form only needs to be completed once.
Please read the waiver and release form carefully as it affects your legal rights in the event of an injury, contraction of a communicable disease, infection, and/or damage to property.
